What To Expect At Your First Visit
General Information
Bourbon Veterinary Hospital is fully handicapped accessible.

For the safety of your pet and our other patients, please bring all pets on a leash or in a carrier. Our clinic is located on IN-331 and is just south of US-30, both of which are highly traveled highways. Additionally, many pets (and even people) are not comfortable with other animals, even if your pet is well-behaved.

Paperwork
For your first visit, we ask that you arrive a few minutes early as we will have brief paperwork for you to fill out. You can also fill out our New Client Form, print it out, and bring it with you.

If you have medical records for your pet from a previous veterinarian, shelter, breeder, or previous owner, please bring these records with you. Alternatively, you can have the records faxed to us at (574)342-6699.

Your Pet's Appointment
During your pet's first appointment we will ask questions about your pet's health history, including:

General behavior

Previous veterinary care, including health conditions, medications, vaccinations, parasite control, surgeries, etc.

Diet

Current health concerns
